Indicate the date attended and the total number of hours attended under the appropriate column. This information will be collected in November and May for submission to the State of Michigan. Please see the attached pages for an explanation of each category. Please return this form to Gayle Lardie by November 9 and then again on May30. If I do not receive a form from you, I will assume there has been no professional development hours earned and a zero will be entered.

Definition of Terms:

SIP – School Improvement Plan. Information study and analysis in small groups of professional colleagues.

MENTOR – Mentoring. Professional development directed to the novice teacher or the mentor teacher.

WORKSHOPS OR CONFERENCES – The number of hours of participation in one-day or short-term professional development which includes LEA’s, ISD, IHE and CENTER.

COURSE – Coursework. Continuing education courses taken for credit at an institution of higher education.

HQS – Highly Qualified Status – Professional development specifically for the purpose of attaining HQS.

ACE – Administrator Continuing Education

HIED – Higher Education

VRTLRN – Virtual Learning
