Please List Any Individuals In Your Group That May Be Involved With Any Other Group That Will Be Performing At The Day Of Music. (This Would Include Any School Ensembles, Rock Bands, Church Groups, etc.) Please List Each Individual’s Name With All Groups In Which They Are Involved With So That We Can Try To Avoid Any Scheduling Complications.

ATTENTION!

All school/church music groups, such as bands and choirs, will be accommodated first regarding staging and performance times. All other performance groups will be scheduled around those times. We will do our best to accommodate everyone’s time request.

All rock bands must submit an audition Tape/CD containing 3 songs, along with lyric sheets for each tune that they are planning on performing at the Day of Music, along with this entry form. Any entry form without these items will not be considered. Please note that not all rock bands will be selected to perform this year due to time constraints.
